By signing below I confirm I have read and agree to the terms of this Custom Package Request including: the selected services above, payment terms (first month due in advance, Net 15 thereafter, $25 late fee), property access authorization, 30-day termination notice, and governing law (Commonwealth of Pennsylvania). I understand that Raven Tide Holdings, LLC handles all invoicing — vendors submit invoices to Raven Tide, which reviews and forwards them to me for payment. I pay vendors directly. Raven Tide does not pay, collect, or disburse vendor payments of any kind.
